Activities Transcript

An Activities Transcript is a free service provided by the Office of Student Life for Mt. SAC students. It documents and verifies College related activities, service learning, and leadership involvement while a student is enrolled at Mt. SAC.

The activities transcript is an opportunity for students to document their involvement outside of the classroom. Each time a students sends for an academic transcript he/she may also request an Activities Transcript through the Office of Student Life.

Below are the steps for requesting an Activities Transcript. Only activities and events requiring substantial planning, implementation, and follow-through should be recorded.

Step 1 -
click here and complete the Activities Transcript form - if you have more than one page's worth of information, make sure to click on the tabs at the bottom left of the file to enter information onto the second page (see below).

Step 2 -
type in the appropriate information, print it out, and bring the completed form to the Student Life Office (9C) for verification and approval
Step 3 -
the form will be returned to you so that you may get the verifying signatures
Step 4 -
Bring signed transcript to the Student Life Office for validation with an embossed college seal

It is the student's responsibility to request that a copy of his/her Activities Transcript be sent with each academic transcript. Please allow five business days for processing.
