Funding Forms
	Request for Funding: Use this form to request that a senator write an appropriation to request funding for your organizations event. The form must be filled out completely and turned into the appropriate senator.
	Appropriation Template: (FOR SENATE USE ONLY) - Use this form to write an appropriation and request funding from senate. This form is only valid if it is written and co-authored by a member of the Associated Students Senate. All other submissions will not be considered.

Application Forms
	Conference Application: Use this form to apply for any AS sponsored conference. All AS sponsored conferences are free to Mt. SAC students who have paid the current Activities Fee ($11 per semester)
	College-Wide Committee Application Use this form to apply to sit on a college committee as a student representative. If you are interested in sitting a college committee, but have questions, contact the Associated Students President at (909) 594-5611, ext. 5956.

	Bill Template: (FOR SENATE USE ONLY) - Use this form to write a Bill and request that senate adopt a new piece of legislation. This form is only valid if it is written and co-authored by a member of the Associated Students Senate. All other submissions will not be considered.

	Organizational Directive: Read this document if you'd like to know more about how Associated Students is structured.
	Publicity Directive: Read this document if you'd like to know how and where your organization can publicize on campus.
	Elections Directive: Read this document if you'd like to know more about Associated Students elections, how they are conducted and how you can run for office.
	Judicial Directive: Read this document if you'd like to know more about Student Court, how it operates, and its structure.

	Activities Transcript: Use this form to document and verify College related activities, service learning, and leadership involvement while a student enrolled at Mt. SAC. After downloading and completing this document see a member of the Student Life Staff for approval BEFORE securing signatures.
	College Committee - Descriptions This provides a brief summary of the college-wide committees that students can serve on.
	College Committee - Meeting Schedules This provides a schedule of the various committee meeting times and locations.
	Marquee Announcement Request Complete this form to have your event announced on the college's electronic marquee.
	Event Planning Checklist This provides a brief summary of the guidelines and forms needed for planning an event on campus.
	Dungeon Guidelines Use this information to familiarize yourself with guidelines for creating posters and other publicity for your club.
	Event Evaluation Use this form to record and evaluate your events and activities. This form can provide useful information and guidance for people who will plan the event the following year/semester.
	ICC Club List This document provides a complete list of the active clubs on campus along with the advisor's contact information.
	Procedures for starting a club This document provides a complete list of directions for how Mt. SAC students can go about starting a new campus club.
	LEAD Application: Use this form to apply for the Leadership, Education And Development program. It's a leadership track, in which students can attend leadership-related workshops and earn a certificate of completion.
	Posting Policies Use this information to familiarize yourself with guidelines for publicizing campus events or activities.
	Travel Release (Student or Staff) Complete this form before attending any College related event that takes place off-campus (e.g. conferences, retreats, field trips, etc.).
